Bruce was killed on the job in August 2013 while working for Ricks Auto Body in Missoula, MT. He was killed when poured lacquer from a barrel to a smaller container through a funnel and the fumes exploded.

Federal OSHA investigated the incident and proposed a $51,100. 00 penalty for eight serious violations. In an informal settlement the penalty was reduced to $36,000.00 for eight serious violations. This case is closed.

peopleGeorge "Junior" Hamner, 54

George was killed on the job in January 2006 while working for Sago Mine, (ICG) in Sago, West Virginia. He died in a mine explosion and suffered carbon monoxide poisoning.

MSHA investigated the disaster and did not identify any contributing violations and assessed no penalties.

----George "Junior" Hamner, 54 died on 1/2/06 at the International Coal Group's Sago Mine in Sago, WV. He and 11 other men died from carbon monoxide poisoning.

Christopher was killed in February 2006 while working at Jennie-O-Turkey in Melrose, Minnesota. He was killed when a piece of equipment crushed him.

Federal OSHA investigated the incident and proposed a $31,000 penalty. The penalty was not reduced and the case is closed.

Monique was killed on the job in

August 2010 while working for Producers Rice Mill in Stuttgart, Arkansas. She was killed when engulfed by rice.

Federal OSHA investigated the incident and assessed a $6,300 penalty for one serious violation. The penalty was not reduced.

---- Monique was found buried in a rice storage facility on Tuesday. The Stuttgart Fire Department received the call about 5:30 a.m. When fire crews arrived on the scene, co-workers were attempting to dig the trapped employee out. Fire crews eventually located Harper. Paramedics tried to revive Harper, but she was pronounced dead at a local medical facility at 6:28 a.m. According to a police report Harper's co-workers became concerned about her whereabouts when they were unable to contact her by radio. That's when employees began searching for Harper. Killed at Producers Rice Mill in Stuttgart, AR

Scott was killed in September 2004 while working at Custom Structural Inc. (iron worker) in Pewaukee, Wisconsin. He died after being struck by a vehicle on a construction site.

Federal OSHA investigated the incident and assessed a $3,000.00 penalty for two serious. In an informal settlement the penalty was reduced a $1,000.00 for one serious violation and one other than serious violation.

Gary was killed on the job in July 2013 while working for Shelly & Sands Inc. in Zanesville, Ohio. He was killed when a dump truck which was loaded with concrete, was backing up when it struck him.

Federal OSHA is still investigating.

Bob was killed on the job in August 2013 while working for City of Minden, Nebraska. He died when he was overcome with methane gas after unclogging a sewer line.

Ryan was killed on the job in May 2009 while working for Diamond Sawing & Coring, L.L.C. at the Nebraska Air National Guard in Lincoln, Nebraska. He died when he fell nearly 20ft while repairing a hangar roof.

Federal OSHA investigated the incident and assessed a $13,300.00 penalty for two serious violations. In an informal settlement the penalty was reduced to a $7,315.00 for two serious violations.

Anthony Holden, 29 was killed on the job in August 2012 while working for Southern Nevada Paving Inc. in Slone, Nevada. He died when was crushed.

Tommy was killed on the job in October, 1996 after working on towers for 35 years. He contracted Acute Radiation Syndrome (ARS) and suffered stochastic effects of radiation.

This is not in OSHA's jurisdiction.

This photo was sent in by his widow Kathy Newman Holley ----Tommy (of Bridge City, TX) passed away from too much radiation exposure working in the tower business. This exposure causes nausea, headaches, and it affects the heart, lungs, and kidneys. He was a “towerdawg” for 35 years. Tommy worked for 11 years on Radio Towers and 24 years on Cell Towers.

Sherman was killed in February 2011 while working at K&K Forest Products in Evart, Michigan. He was killed when struck by a felled tree.

MIOSH investigated the incident and assessed a $1,525.00 penalty and 3 serious violations.
There were no reductions.

As his daughters we feel his biggest pride was his grandchildren. He promised them he would teach them to hunt and fish when they were older and sadly he never had the chance. Our dad was with us when and where ever we needed him. He traveled hours for his first grandchild Leslies open-heart surgery and as she passed he was with his family. He made the trip to Ohio when his next two granddaughters Lauryn and Kynndal were born, and to Alexandria Virginia when his first Grandson Jackson was born. He also made the trip when Granddaughter Gabriela was born and was the first one to the hospital when she had her first open-heart surgery. Danielle was still pregnant with the youngest granddaughter Jillian, when dad was killed. ----If asked to describe one of our fathers greatest joys (besides family) we all agree coffee and a doughnut would be a top priority. Our lives will never be the same and we miss him everyday."

Darrel was killed on the job in July 20, 2008 while working for Allstate Tower Service in Petersburg, North Dakota. He fell 50 feet while painting a tower.

Federal OSHA investigated the incident and proposed a $ 9,000.00 penalty for six serious violations. The fines were paid and the case is closed.

Jonce was killed on the job in July 22, 2010 while working at Mccord Communication Inspection (3rd party Barnhart Crane and Rigging Co., Inc.)  in McClellan, Al. He died when a tower collapsed after a bucket truck struck a supporting wire. 

Federal OSHA investigated the incident and proposed a $ 7,000.00 penalty for one serious violations to Barnhart Crane And Rigging, Inc. In an informal settlement the penalty was reduced to $3,500.00 for one serious violations.  The fines were paid and the case is closed.

This photo was sent in by his widow Gettie Hubble.  -----Jonce Hubble was doing maintenance on a communications tower in Anniston, AL on 7/22/2010. The tower's guy wires were clipped by a 33 foot bucket truck that was moving around the worksite. Jonce passed away around 9.00 pm at UAB in Birmingham after several surgieries and two amputations.

Butch was killed on the job in August 2011 while working for Union Pacific Railroad in Saline, Kansas. He died when his truck became stuck on the tracks and a car struck him.

This photo was sent in by his widow Tami Lynn Hulse. ----Butch was killed while working at Union pacific Railroad as he was on the rail truck inspecting. He was going through a crossing when a woman hit him knocking his truck off the track, the truck rolled and he was thrown from it.

Ossie was killed in October 2011 while working for Port Of Miami River Terminal, LLC. in Miami, Florida. He died when the platform he was standing on failed, he fell 18ft. and a container crushed him.

Federal OSHA investigated the incident and proposed a $14,700 penalty and three serious violations. In an informal settlement the penalty was reduced to $11,025.00 for one three serious violations. The company paid the fines and the case is closed.
