Kevin Scott Noah, 42
Kevin was killed in August 2001 while working for Himmick/Obayashi/Golden Gate Bridges Seismic Retrofit in San Fransico, California. He died when he fell off the Golden Gate Bridge.
CalOSHA investigated the incident and proposed a $26,025.00 penalty for three serious violations and three other than serious.
In an informal settlement the penaltly was reduced $0.00 and no violations.

Tommy Osier, 18
Tommy was killed while working for Pine Groves Farm in Michigan. He was killed when he was ingulfed in grain.
MIOSHA investigated the incident and assessed a $7,00.00 penalty and one serious violation. In an informal settlement the penaltly was reduced $3,500.00.

----Today is the 3rd month without my “baby” boy. Tommy was entrapped in an old corn silo that a farmer had
insisted in using for three years without a bottom feed unloader. Young men were required to climb in and shovel the corn down to
an external auger to feed cattle. The “boys” 16-25 yr olds, took turns climbing in for 5-10 min at a time switching out when the
gasses were to strong. Tommy had milked that morning leaving home at 3 am, since it was Memorial Day The farmer wasn’t going to
work and Tommy’s replacement was a no-show. Tommy was told “stay and keep your job – Leave and don’t come back.” We had family
birthday plans with Tommy since he had just turned 18, 14 days earlier. He was trying to get out as quickly as he could. The
corn had created a cave, he hit the corn with the shovel and it came down on him funneling him to the bottom outlet 15 feet below,
folding his body in half at the waist, entrapping his lifeless body in the small opening. I am convinced that the Lord swiftly
released him from his earthly form. It took rescuers 5 hours to recover his body from the silo.

Alex Pacas, 19
Alex was killed in July 2011 while working for Haasbach LLC. in Mount Carroll, Illinois. He was engulfed by grain. Wyatt Whitebread also died in this same
OSHA investigated the incident and proposed a $555,000.00 penalty
for willful violations, serious violations and one other than
serious violation. In an informal settlement the penalty was reduced $200,000.00
for twelve willful violations, twelve serious violations and one
other than serious violation..
There was and additional penalty from Wage and Hour of $68,235.00.

Jay Paul, 48
Jay was killed in June 2008. He died when he was caught in a large gantry machine.

Bradley B. Pederson, 35
Bradley was killed in June 2010 while at work. He died in a truck roll over.

----My 35 yr old son Brad was killed 2 months ago in an truck rollover accident while on the job. He was hauling wood chips and debris cleanup from a logging project. Dante M. Perales, 30
Dante was killed in February 2005 while working as a Police Officer in Orlando, Florida. He died after he was shot.

----His name is Dante Perales and he is loved and missed more than words can ever say. Everyone seen the
picture of him in his Police uniform but I wanted to show a different photo of him. I love you Dante and would do anything
to have you back. Love Patricia. He died from a gunshot wound.

----My brother was just 37 years of age...he had a very full life but I am sure he was
looking forward to enjoying much more of it for many years to come. If we, as a family,

Gregory Piper, 61
Gregory was killed in October 2011 while working for Boat Grotto in Santiago, California. He died with a Pulmonary Aneurism which was the direct result of a workplace injury.
Cal/OSHA investigated the incident and proposed a $260.00 penalty for two other than serious violations. There was no reduction.

Billy was killed in May 2008 while working for Tri-County Roofing/Bryan Phillips Construction in Malvern, PA. He died from a roof fall.
Federal OSHA investigated the incident and proposed a $2,000.00 penalty for one serious violation.
In an informal settlement the penaltly was reduced to $1,400.00 for one serious.

----The subcontractor Tri-County Roofing, contractor Bryant Phillips Construction. Both companies were
issued serious violations from OSHA for no safety equipment and proper training.
Dallas Prokopis, 31
Dallas was killed in October 2011 while working for Greene's Concrete Cutting in Utah. He died after being struck by a dump truck.
UOSH investigated the incident and proposed a $3,000.00 penalty for one serious violation. They were not reduced.

----Dallas was moving traffic barrels and due to miscommunication between workers the road closer was opened for traffic. A dump truck
came through and hit my husband. He was taken to the hospital and after five days we had to take him off life support, he sustained severe head
trauma and a lot of broken bones. I was 8 months pregnant with our third son when he died. Every day is a struggle for me I miss him so much!

Gary was killed in August 2001 while working for Miadville Redi-Mix Concrete in Meadville, Pennsylvania. He died from a fall.
Federal OSHA investigated the incident and proposed a $6,000.00 penalty for two repeat violations. These were not reduced.
